How to Create a Check Box Form Field in Adobe InDesign

Any rectangular frame with unassigned content can be turned into a check box. There are also pre-formatted check boxes in the sample buttons and forms that come with Adobe InDesign.

This feature works the same in Adobe InDesign CS6 and CC versions.

  1. In the Window menu, go to Interactive and choose Buttons and Forms.Buttons and Forms in Window Menu
  2. Go to the Buttons and Forms Panel menu and select Sample Buttons And Forms.Sample Buttons And Forms in Buttons and Forms Panel Menu
  3. Click and hold on the sample check box you wish to use in the Sample Buttons And Forms Panel, and drag it into the layout. Resize the check box in the layout as needed, making sure to hold down the Shift key as you drag at a handle.Check Box Added to Layout
  4. The object is already formatted as a check box. To set the options and control the default appearance, go to the Buttons and Forms panel. Give the check box a unique name, then go to the Appearance section to set the default appearance.Check Box Options in the Buttons and Forms Panel
  5. The file must then be exported as an Interactive PDF.
